Our format is a proven formula to improve response time and make it easier to submit a ticket. We'd like you to take action to:

1. Create a ticket in Line format with 1 change per line.
2. The 11 digit DID please no spaces dashes dots or parenthesis
3. Model # and 12 digit MAC address no spaces dashes dots or colons
4. Good contact number if we need more information

Here is an example of a format we would like to see when tickets are being submitted:

Can you reset the voicemail PIN for XXXXXXXXXX?

Name is John Brown

Email address [e-mail]

Model: 8851

optional Mac address (last 4) AABBCCDDEE22

• Reset the voicemail PIN

• Remove any existing email addresses used for voicemail notifications
